If an independent contractor hired by Financial Action, inc. in the assistance of your planning, we will request that disclosure be approved by you using a Consent to Disclose form naming such contractors.

Financial Action, inc., will not disclose any personal information about you or your account(s) unless one of the following conditions is met:

• We have documentation that the recipient is your authorized representative; or
